Affixed Labels

By affixing one or more labels to a form – usually a continuous form – we are able to create a more versatile form that can be used for multiple purposes.  In addition, after affixing, the labels – whether printed or blank – can be consecutively numbered and consecutively barcoded.  Label material can be permanent or removable adhesive and can have standard or imaging liner.  Depending on customer needs, the label materials used may be those designed to be used on specific substrates, i.e., test tubes, corrugated, etc.

Common affixed label applications include:

  • Direct Mail
  • Bill of Lading and Shipping Labels
  • Inventory Control Forms
  • Hospital Lab Requisition Forms
  • Chain of Custody Forms

Affixed label form users may include:

  • Distribution Centers
  • Mail Order Houses
  • Hospitals
  • Laboratories
  • Pharmacies
  • Financial Institutions
  • Record Storage Houses
  • Retail Outlets
